Perhaps the worst written, and certainly the most overwritten book I have ever read. The author sounds like a seventeen year old with a serious case of verbal diarrhoea. In compensation, though, it is thorough, with lots of notes, many of which ought to have been incorporated into the text, such as numerous tables. There was a lack of graphs and diagrams, and the maps that were included were improper for their purpose, in the wrong place in the text, and there were too few of them.
The best parts were the last two chapters, and a quote at the top of page 506, which should be copied out and pasted to the top the desk or edge of the computer screen of all health care administrators, policy makers, and political critics.
